Registration on Saturday will be at the registration tent outside Sandwich High School. If registering on Saturday, please be aware that the registration tent location may not be the same location that your team will play its first game (there are 4 different field locations). If this is the case, plan to register your team at least 1 hour prior to your first game to allow time to travel to the designated field.
Please Bring the Following With You for Registration:

Two team rosters (we keep one) that have been signed by your club registrar, as well as your state or league registrar.

Player Pass Cards or Birth Certificates and a PHOTO ID for EACH PLAYER. This is mandatory. Unfortunately we have had teams in past years that have had older players on their teams pretending to be a younger member of the team. ANY AND ALL of these instances will result in the club, the coach, and the player being reported to their state youth soccer association. Referees will be checking rosters.

Medical Release Form (PDF Format)
*Must be signed by all parents prior to tournament*

Permission to Travel Form (required for Non-Region I States only). If you are not from a US Youth Soccer Region I State (see http://www.usyouthsoccer.org/, click on "State Associations" and then on "Region I website" for list of Region I states), you are required to complete and submit attached form.

Coaches are requested to provide their cellular phone number and/or hotel phone number
(if applicable) to the tournament registrar at the time of Registration. This is important if we have to reschedule a game during the weekend due to field or weather conditions, or to notify you about the final round scheduled for Monday May 25, 2009.
