WAIVER/RELEASE COVID-19

ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT

In consideration of being allowed to participate on behalf of Sandwich Soccer Club athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

Participation includes possible exposure to and illness from infectious diseases including but not limited to COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,

I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, if I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Sandwich Soccer Club their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.

Sandwich Soccer Club Fall Guidelines

No more than 25 players on any playing surface/area for group sports.

Coaches, staff, referees and other officials are required to wear facial coverings.

Facial coverings must be worn by all soccer players EXCEPT: during high intensity activities, when distancing of 6 feet or more is possible between players or for individuals that are unable to wear a face covering due to a medical condition. 

Each player must provide their own water bottle, face covering and hand sanitizer. Only a face covering which attaches around the ears will be allowed, so as not to cause injury if accidentally tugged or pulled on. No “gaitors” allowed. Each player should bring both home and away jerseys (or light and dark t-shirt) to practice and games as practice jersey’s will not be provided. 

Players should not arrive at their field earlier than 5 minutes prior to their scheduled arrival time and must leave immediately after their session. Please use the marked entrance and exit signs at the field.

Players & Spectators: Check for normal temperature before going to training, if higher than 100.4, please stay home. No signs or symptoms of COVID-19 in the past 14 days and have not been exposed to someone who has been ill in 14 days. Players must come from a low risk state in order to participate without quarantining. Do not attend practice or game if you are in a high risk category.

One spectator per player (defined as parent, legal guardian or chaperone).  Exception: any spectator who is responsible for other minors and not able to leave them to be cared for by others, such children must be with the spectator at all times. 

Spectators must wear facial coverings and maintain 6 feet of social distance at all times

All spectators must provide their name/phone number/email to team-parent volunteer for contact tracing if needed. 

Spectators must keep at least 6 feet away from the playing surface.

SANDWICH SC RETURN TO SOCCER ACTIVITIES COMPLIANCE / IMPLEMENTATION PLAN

Under its Return to Soccer Activities Compliance / Implementation Plan (“the Plan”), the Sandwich Soccer Club (“SSC”) intends to follow to the greatest extent practicable and possible the Massachusetts Youth Soccer (“MYSA”) Return to Soccer Activities Guidelines (“the Guidelines”)(as issued and updated on June 12, 2020). The following Plan elements are drawn from these guidelines and are intended to identify responsible parties from the SSC Board that have been assigned on June 14, 2020, to specific tasks and to provide the set of initial steps that SSC has or is undertaking as part of return to play process under the MEOA Phase II. SSC reserves the right to modify this compliance plan as needed in the interest of SSC player, coach, and SSC Board member safety.

  • D. Michael Ball, in his capacity as a SSC Vice President of Player and Coach Development, is designated as the SSC COVID19 Safety Officer;

  • Adam Tedeschi, in his capacity as SSC Vice President and Treasurer, shall be responsible for COVID19- related communications between SSC and the Town of Sandwich Board of Health;

  • Tim Baker, in his capacity as SSC President, shall be responsible for coordinating adequate equipment cleaning and/or sanitization at the SSC facility (Bob’s Fields / Water District Fields). Frequency of facility and equipment usage will determine frequency of cleaning / sanitization;

  • SSC will post and maintain signage that will identify entrance and exit pathways to and from facility practice fields;

  • SSC will post suitable signage at the facility recommending proper hygiene practices in the age of COVID19 and other communicable diseases;

  • SSC will be eliminating and/or limiting the use of share practice equipment until further notice;

  • SSC will post the MYSA Guidelines on the SSC website and through SSC-run digital media;

  • In its effort to promote participant health and safety, SSC will provide all current SSC age group directors and coaches with the MYSA Guidelines and other related documents (and/or a digital link to these documents) and require that these documents be provided to player parents;

  • SSC will schedule all team practice day and time assignments and will assign practice field space in order to maintain MYSA-recommended player group spacing at the facility;

  • SSC will ask all player parents/guardians to minimize the amount of time they are present on or near the practice field portions of the facility. SSC has improved visibility between the parking areas and the fields in order that parents are more comfortable with maintaining distance from player groups during practice sessions;

  • SSC expects players, coaches, parents/guardians, and other visitors to the facility to wear a face covering at all times except while participating during small-group training sessions;

  • SSC is requiring coaches to maintain a record (written or digital) of all practice session attendees;

    If enhanced steps/protocols are necessary in the interest of participant health and safety, these steps/protocols will be added to this document and revised/updated versions of this Plan will be provided to the South Coast Soccer League Board of Directors
